How
to Make a Referral
You can refer an employee
in one of two ways:
Informal Suggestion
1. Suggest that employee
contact EAP/MAP.
2. Explain EAP/MAP counseling services.
3. Give handout or brochure and the phone number to call to schedule.
Supervisor Referral
1. Contact St. John's Mercy EAP/MAP
- Discuss nature of problem
with EAP/MAP Counselor.
- Discuss recommendations
regarding employee.
2. Prepare to Meet with
the Employee
- Gather documentation
and note important points to be discussed with the employee.
- Determine your goals
for confrontation.
- Anticipate employees
responses.
3. Meet with the Employee
- Find a private place
to meet.
- Be firm, formal and concerned.
- Present and review documentation.
Stick to work performance.
- Explain in specific terms
why their performance is unacceptable.
- Establish an acceptable
level of performance.
- Obtain a commitment from
the employee for a plan action.
- Establish a time frame
for the next performance review.
4. Refer the Employee
- Offer the company resource,
the St. John's Mercy EAP/MAP.
- Express support from
the program and the concept of EAP/MAP.
- Explain and emphasize
confidentiality and the fact that there is no cost for EAP/MAP services.
- Give employee information
on how to contact to St. John's Mercy EAP/MAP.
- Encourage employee to
utilize all available resources to upgrade unacceptable performance.
- Stress that it is the
employees decision whether to seek assistance.
